I hereby authorize and grant permission to Communications LAB and its affiliates, agents, and representatives (collectively, the "Company"), to use my name, voice, likeness, image, and any other attributes of my personality (individually and collectively, Talent’s “Likeness”)in connection with the production and exploitation of any and all audiovisual works, photographs, recordings, and other media produced by the Company (the "Production").

I understand and agree that my Likeness may be used in whole or in part, edited, adapted, or modified, and used in any medium or context, including without limitation, theatrical, broadcast, cable, satellite, and internet transmission, and in any and all promotional and advertising materials for the Production. As the sole consideration for the grant of rights hereunder, I acknowledge my participation in the Production is unpaid.

I hereby represent and warrant that I have the right and authority to grant the foregoing rights, and that my participation in the Production will not infringe or violate any intellectual property rights, privacy rights, or other rights of any third party.

I acknowledge and agree that the Company shall own all rights in and to the Production and any and all materials created in connection with the Production, and that the Company may exploit such rights in any manner it deems appropriate, without any further compensation or attribution to me.

I hereby release and discharge the Company and its officers, directors, employees, agents, and affiliates from any and all claims, demands, or causes of action that I may have against them arising out of or in connection with my participation in the Production.

I understand and agree that this Talent Release Form shall be binding upon me, my heirs, executors, administrators, and assigns.
